Notre Dame Long Term Care and Rehabilitation Center offers quality care in a warm, home-like environment that is anchored in spirituality and delivered by a team of caring, compassionate, and dedicated staff who embrace a resident-focused philosophy of mind, body, and spirit.

Memory Care

Cognizant of the special needs of those with impaired memories, we developed Harmony Village, a unit designed to provide for the physical, social, cognitive, and emotional needs of our residents. Our goal is to help each resident realize their greatest potential cognitively, physically, psychosocially, and spiritually. To achieve that goal, we provide a caring and secure environment with specialized personal care, spiritual support, and meaningful, success-driven activities.


A specially-trained, experienced, inter-disciplinary health care team creates an individualized plan of care that aims to address the unique needs of each resident. All staff is trained to work with individuals who have Alzheimer’s and other dementias, and some staff have been certified by the National Council of Certified Dementia Practitioners (NCCDP). We offer all staff the opportunity to become Certified Dementia Practitioners™.


Our care teams in Harmony Village offer services in a calm and soothing environment. Residents are able to engage in group activities, one-on-one activities and sensory activities. It’s common to smell the aroma of fresh baked cookies and other familiar scents of home. We encourage each resident to use life skills and each program is adapted to their individual abilities.

At the entryway to each resident’s room is a memory box that contains mementos and items of importance to both the resident and family members. These special boxes help provide a link between the past and the present. All the amenities in Harmony Village are designed to meet each resident at his or her level.

  • Training for employees to become Certified Dementia Practitioners™
  • Weekly staff meetings to communicate and update individualized resident care plans

  • Aroma Therapy and Music Therapy to alleviate symptoms

  • Memory Boxes with pictures and mementos outside each resident room to give staff, families, and residents a glimpse into their life and provide a connection between the past and the present
  • “Day Trip” for residents potentially transferring to Harmony Village, giving the resident, their family, and the unit team an opportunity to interact prior to transfer
