Notre Dame Health Care is nestled in a peaceful, quiet wooded area on Plantation Street on a site originally named “God’s Acre” by the Sisters of Notre Dame de Namur.

In addition to the gardens and courtyards, the campus includes buildings for du Lac Assisted Living Residence, including our Harmony Memory Care community, our Long Term Care and Rehabilitation Center, including our Harmony Village (Memory Care), and administrative offices for our Adult and Pediatric Hospice and Palliative Care Programs.
