Notre Dame Affinity Care Management is here...
Notre Dame Health Care Center is pleased to announce its new care management program Notre Dame Affinity Care Management. This program provides premier services to assist elders, the disabled, those struggling with complex medical issues and their families. Beginning in April, Notre Dame Affinity joins the professional services from Nancy Johnson's former Affinity Care Management practice and the Notre Dame mission to "demonstrate concern for the whole person in caring for the spiritual, social, physical and psychological well-being of our residents, their families and the community we serve."
