Adult & Pediatric Home Hospice Care Services
Notre Dame Hospice Care provides comfort, compassion, companionship and support to patients nearing end of life and their family members when a treatment or cure for an underlying disease is no longer available. Services are delivered by a dedicated team of health care professionals who focus on addressing physical, mental, social and spiritual needs to improve quality of life.
At Notre Dame Hospice, we provide 24-hour, on-call availability to provide comfort and peace of mind to the patient and their family.
Plans of care are individualized to enable each patient to navigate through end of life with hope and meaning while establishing their goals of care. In line with Notre Dame Health Care’s mission of having a holistic approach to care, mind, body, and spirit are all critical aspects of each individual’s journey. We not only assess and manage all physical symptoms related to the illness and end of life, but also the emotional, spiritual, and social aspects of coping with a terminal illness. Special attention is paid to honoring the specific needs and customs of all ethnic backgrounds.
Notre Dame Hospice also participates in the We Honor Veterans program developed by The National Hospice and Palliative Care Organization, in collaboration with the Department of Veterans Affairs.
Services Include:
- All items and services needed for pain relief and symptom management
- Physician, nurse practitioner, and nursing services
- Social services and assistance in navigating the complexities of end-of-life decisions, with resources and education on advanced directives, Medicaid applications, and more
- Medications related to terminal diagnosis
- Durable medical equipment for pain relief and symptom management
- Aide and homemaker services
- Music and massage therapy
- Volunteer services
- Other covered services you need to manage your pain and other symptoms, as well as spiritual and grief counseling for you and your family.
- 24 hour a day/7 day a week on-call availability of a qualified hospice professional
