Notre Dame Health Care has been honored and blessed to have received some very thoughtful and generous gifts from individuals who made plans to support our work beyond their own lifetime. We are humbled by these very thoughtful contributions.
Gifts made through a will, trust or beneficiary designation can have a tremendous impact at Notre Dame Health Care without affecting your current financial resources.
You can make a bequest or gift through your estate by including a provision in your will or estate planning directives, or by naming Notre Dame Health Care as a beneficiary of your life insurance policy or retirement plan. The amount left to the Notre Dame Health Care (or any charity or heir) can be expressed as a dollar amount or as a percentage of the residuary estate.
If considering a gift of funds through your estate, the following language may be helpful to assist you and your attorney:
“I leave the amount of $______ (or ___% of my residuary estate) to Notre Dame Health Care Center, Inc., a Massachusetts not for profit corporation, E.I.N. 04-3108782, with an address of 559 Plantation Street, Worcester, Massachusetts, for its general purposes.”
